I've tried creating a new campaign, but still no luck. I am basically out of ideas on what could be going wrong.

There was something weirdly wrong with the definition.xml file. although it all looked correct, it wasnt loading properly. I recreated it from a blank file and it loaded just fine. I did make a few minor text tweaks. Try this copy.

I had originally created the files and the .mod using the examples included in the guide. Perhaps that is why it wasn't working. Either way, thanks a ton for your assistance it is working now in my campaign
