Affliction powers (and conditions)

Does someone have an example or how-to for building an Affliction power for a PC?

In HeroLab it's this:

Deathly Illness: Affliction 8 (16 PP)
Nausea, Vomiting, 1st degree: Impaired, 2nd degree: Defenseless, 3rd degree: Incapacitated, DC 18;
Alternate Resistance (Dodge), Increased Range: ranged (Standard - Ranged, 200/400/800 ft. - Instant)

Do I just freeform type all that stuff into the power block?

And while I am on the topic, is there any way to add an indicator to the characters in the combat tracker so I can at a glance remember that they are stunned or dazed or whatever?
