I just opened up the Story section of my campaign and noticed that all of the groups had been dropped. All of my story entries are now in the single default group.
Not sure what caused this, just letting you know that it happens :)

Did this happen spontaneously or were you doing something specific such as importing a FG1 campaign to FG2 when it happened?
I managed to reproduce this bug. It happens if you run FG as a DM, and during the course of the session, you never open a particular section (story, images, etc.). Any of the sections that you don't open at least once during the session will have all their groups dropped when you exit.
I had noticed the combining of tabs also a few versions ago, but couldn't figure it out. If you do only have one tab in a book and it is not the default red one, it changes it back to the default red one.
