Has anyone come up with an automated way to handle this? I have added the effect to do the extra damage and have them turn it off unless they roll a natural 20. Is that currently the only way to really do it.

You can add an effect of DMG: ndn, critical (I can’t remember what the extra damage is but substitute that for the ndn bit). This will however work for any weapon not just the sword. You could use this extension to limit the extra damage to only the sword.

so how do you avoid this when a person has a crit of 19?
You can’t in any automatic way since if you set the critical range to 19-20 then FG will see that as a critical and add in the extra damage. There isn’t an effect that will fire only in a 20.
It's not automatic, but we just created an effect that does 4d6 slashing (I think that's the amount). Any time our Conquest Palidan (the guy with the sword) rolls a nat20, I just have him drag and drop the damage on the target. We've also written it like the rogue's Sneak Attack damage, where he applies it to himself any time he rolls a nat20, but before he rolls damage.
I know this doesn't answer the question at hand, just throwing out alternatives since it can't be automated.
Just as an FYI to you, the SRD seems to have the # set at 4d6 for the nat 20; but in the rulebook it is actually an extra 14 slashing damage and quoted here in Sage Advice https://www.sageadvice.eu/2018/01/25...ble-on-a-crit/
